Further I understand and agree that this requires my participation. I understand if I fail to meet acceptable levels of participation I may be dismissed. I also agree that ALL equipment, radios etc. is the property of Chestnut Hill Volunteer Fire Department and must be surrendered upon dismissal.

I further understand that Chestnut Hill Volunteer Fire Department conducts a background check of its membership for the purpose of protecting the public we serve. I hereby shall agree to such a background check before membership will be granted

I hereby authorize Chestnut Hill Volunteer Fire Department and its designated agents and representatives to conduct a comprehensive review of my background causing a consumer report and / or an investigative consumer report to be generated for employment and or volunteer purpose. I understand that the scope of the background check may include, but not limited to the following areas: verification of social security number, credit reports, current and previous residence, employment history, education background, character reference, drug testing, civil and criminal history records from any criminal justice agency in any or all federal, state, and county jurisdictions, driving records, birth records and any public records.
